The 2012 Central Division Tournament dates and registration information flyer is posted below.  Our events will be independent events this year and are open to all teams regardless of affiliation (USAV, JVA, AAU, etc.).  We want our events to remain a viable option to all clubs/teams regardless of the association they choose.  All events are fully insured.  Please click on the link below to view/print the forms.  If you have any questions, please do not hesitate to contact us.  We hope to see you at our events this season!

2012 Central Division Tournament Series Dates and Registration Information

Club Directors/Coaches:


For the upcoming Central Division Events at Illini Elite you will need to have medical waivers and release of liability for all players participating.

 

If you are using JVA Insurance please make sure that your club is registered with the JVA and has all the forms on allplayers.com or with the coach. For these teams you will be receiving $25 back per team that is entered in each of our events. Please let me know if you are a JVA registered club so I know to refund the right amount to you.

 

If you are not using JVA insurance each coach will need to have each players' medical forms on hand the entire time during the tournament. The USAV medical forms are acceptable if you are a USAV registered club. Each coach will then have to sign the JVA Event Sign-In Form stating that all players/parents have filled out the JVA Release of Liability form. You will not turn these forms in, but the coaches will sign a form stating they have them all. You can download a copy of the JVA Release by clicking the link below:

PLEASE - EVERYONE READ THE FACILITY RULES BELOW:
  NO OUTSIDE FOOD IS ALLOWED IN THE ILLINI ELITE CENTER
  NO FOOD OF ANY KIND OR ANY GUM IS ALLOWED ON THE COURTS
  PLEASE DO NOT BRING ANY VOLLEYBALLS INTO THE FACILITY.  WE WILL PROVIDE BALLS FOR WARM-UPS FOR ALL TEAMS.
  ONLY RECLOSABLE DRINK CONTAINERS ARE ALLOWED ON THE COURTS
  ADMISSION FOR EVENTS IS $3.00 (JUST AS IN YEAR'S PAST)
  CHAIRS ARE PROVIDED FOR SPECTATORS
